I understand that my account will be billed $5 per month until I notify Westside Tan. Upon reactivation of my membership, a pro-rated amount of my monthly dues must be made. Regular payments will resume on the first of the month following reactivation. Upon reactivation, reimbursement of my Stay & Save fees will be issued to me via a credit on my account which may be used towards any in store purchase (excluding tax) or Membership Payments.

    Membership Freeze requests must be submitted by the 20th of the month in order to Freeze Membership for the upcoming withdrawal. For requests submitted after the 20th, the Membership Freeze will take effect on the following month.

    Freeze form submitted BEFORE January 20th: Freeze will take effect on February 1st
    Freeze form submited
    AFTER January 20th: Freeze will take effect on March 1st

    Membership Freeze requests take effect on the 1st, allowing access to services through the end of the month.

    Note: Submission of this form does not guarantee Membership Freeze. Customers must be eligible for Membership Freeze in order for the request to be processed successfully. A Guest Services representative will contact you within 3-5 business days to confirm your Membership Freeze. Membership Freeze requests are based on the date of form submission. If you do not receive confirmation before the 20th of the month but your form was submitted before the 20th of the month your request will still be honored.


    On January 1, 2023, all membership rates were increased. Any membership purchased prior to January 1, 2023 will keep their contracted rate but are no longer subject to Freeze Membership. If aforementioned members would like the option to freeze, please visit any Westside Tan salon or let us know in the email below that you would like to upgrade to the new membership pricing.     Membership Freeze requests are not finalized until you receive Membership Freeze Confirmation from a Guest Services Representative.
