Acknowledgement

I understand that my account will be billed $5 per month until I notify Westside Tan. Upon reactivation of my membership, a pro-rated amount of my monthly dues must be made. Regular payments will resume on the first of the month following reactivation. Upon reactivation, reimbursement of my Stay & Save fees will be issued to me via a credit on my account which may be used towards any in store purchase (excluding tax) or Membership Payments.

Membership Freeze requests must be submitted by the 20th of the month in order to Freeze Membership for the next month. For requests submitted after the 20th, the Membership Freeze will take effect for the following month. Membership Freeze requests take effect the 1st, allowing access to services through the end of the month.

Note: Submission of this email does not guarantee Membership Freeze. Customers must be eligible for Membership Freeze in order for the request to be processed successfully. A Guest Services representative will contact you within 24-48 hours to confirm your Membership Freeze.

Membership Freeze requests are not finalized until you receive Membership Freeze Confirmation from a Guest Services Representative.
